Parks and Recreation Program Evaluation

The Glastonbury Parks & Recreation Department is pleased to have you participate in our programs. We recognize the importance of providing the highest level of service to Town residents, and would appreciate hearing from you concerning your experience in specific programs and/or activities.

At the completion of most programs, an evaluation is distributed. If you have already completed one, THANK YOU! If not, and you would like to provide input on a program you have recently participated in, please follow the instructions below to submit an online evaluation:

1. If you wish to print and mail or print and submit the form in-person, click the Program Evaluation link, complete the form, then click the “Print Form” button on the form.

2.  If you wish to complete the form online, click the Program Evaluation link, complete the form, then click the “Submit by E-Mail” button on the form. If you are submitting by email, you will be prompted with the options below after you click the submit by email button. Select the appropriate option and follow the instructions.

• Desktop E-Mail Application: Choose this option if you currently use an e-mail application such as Microsoft Outlook Express, Microsoft Outlook, Eudora, or Mail.

• Internet E-mail: Use this option if you currently use an internet e-mail services such as Yahoo, Hotmail or Comcast.

• Other: Choose this option if your preferred desktop application is not available or you do not know which option to choose.
